This presentation will provide attendees with an illustration of how societal health needs can be met within and outside the walls of their physical therapy practice including: current review of literature on social determinants of health and its impact on physical therapy care, population health and its application in an outpatient clinical practice operating in a traditional health care system, addressing health disparities through physical therapy in underserved and multicultural communities, exploring primary provider role in community practice and community building approaches. The presentation will be delivered in a variety of learning methods including lecture, case presentations and group discussions.
